FPCLG Service Camp Permission Slip

This online form will serve as your explicit permission for your child(ren) to participate in the June 8-12, 2026, summer service camp of the First Presbyterian Church of La Grange (FPCLG). This includes permission to be driven (by a Trained Adult Leader from FPCLG) to the food pantry in Brookfield on Tuesday and to Helping Hand in Hodgkins on Wednesday, and to walk to the La Grange Theatre on Friday.
